A Clinical Review of 120 Cases of Acute Cholecystitis

Abstract
Although the subject of acute cholecystitis already has been comprehensively reviewed at various times, there are certain aspects of the diseases that will bear emphasis.
A study has been made of 120 cases of acute cholecystitis treated surgically and mdedically at Dept. of General Surgery of Pusan University Hospital from January 1958 to July 1968.
Out of 120 cases of acute cholecystitis, 30 cases were treated medically, 10 cases of these were recurrented and treated surgically, 100 cases (90 cases expect 30 cases treated medically, and 10 cases recurrented from medical treatment) were treated surgically.
The following results were obtained.
1. Acute cholecystitis was most common in biliary diseases and its incidence was 0.45% in these series of cases.
2. The sex distribution assumed a ratio of 1.79 males to 1 female, and the yongest case was 6 year old, the oldest case was 75 year old and average was 39.3 year old.
3. 67 cases (55.8%) out of 120 cases gave a previous history of gallbladder diseases.
4. The most prominent symptoms and signs were pain in the R.U.Q. (80.83%), radiating pain to the right shoulder (45%), intermittent colicky pain(53.3%), Fever over 100 F (55.8%) indigestion (37.5%), tenderness on the R.U.Q. (71.6%) and jaundice (25.8%). These were significant findings in diagnosis of acute cholecystitis.
5. The majority of the patients (56 cases) were performed early Operation within 7 days. Of the 100 cases studied by us cholecystectomy were carried out in 38 cases, cholecystectomy with choledochostomy in 45 cases, cholecystectomy with choledchostomy in 4 cases, cholecystectomy with choledochostomy and transduodenal sphincterectomy in 5 cases, and laparatomy and drainage in 3 cases.
6. Possible etiologic factors were cholelithiasis (75%), ascaris (10%). clonorchis(5%), typhoid fever(2%) and liver absecess (1%).
7. The mortality rate in this series of cases was 9.166%.
